Our client is a Hospital and Health Care company that provides virtual behavioral health, and on-site pain management, physiatry, and other specialty care for adults living in skilled nursing facilities and other congregate living communities. Their Virtual Behavioral Health Clinic, supporting all adults and caregivers, is now open to anyone 18 and over.

Required Skills and Experience: •Empathy and a genuine interest in providing care for a geriatric population •APRN or PA-C Texas license •Strong interpersonal skills as you will interact with many team members at the company and partnering skilled nursing facilities •Bilingual in English and Spanish preferred •Post-acute care experience either in a nursing home or acute rehab setting. •Specialty certification in Gerontology is a plus Benefits: •30 days of paid time off (PTO) per year, up to 10 of which can roll over to the following calendar year (note that this will start in 2022; there are no set days for the remainder of 2021) •12 weeks of fully paid parental leave •Competitive medical, vision, and dental health plans •401(k) plan through Human Interest, including an employer match of 1% of annual salary •Stock option grants for all team members Additional Job Details: •Conduct in person visits with patients in skilled nursing facilities. •Work with Physical Medicine and Rehab physicians to develop care plans and treatment recommendations for pain management and the improvement of functional and behavioral health in the geriatric populations. •Attend weekly team meetings at skilled nursing facilities to help coordinate care and make treatment recommendations. •Perform joint injections and botulinum toxin injections for spasticity as needed.
