Registered Nurse New Orleans, LA Full Time Are you a Registered Nurse who is passionate about leading effective teams and working with the chronically ill? We would love to meet you! We are looking for someone to join our team that is revolutionizing the delivery of healthcare to vulnerable patients. As a Health Services Director, you will oversee the day-to-day operations, development, and implementation of care planning through the interdisciplinary care team who are delivering care in the home of patients with complex, chronic diseases. You'll also manage relationships with vendors, health plan case management, community-based providers, and community organizations. This includes establishing processes and oversight for the in-patient, SNF/LTC and post-hospital discharge programs.
