We are looking for a Care Coordinator RN to join our team in a case management position. This RN will spend most time within the community driving to patient homes and/or mutual locations. All RN Care Coordinators participate in a 30 day orientation period at our offsite location, and here they learn the essentials of the work and documentation of case management. During the orientation process it will be determined by the leadership team, the care coordination team and the RN Care Coordinator where the best fit for his/her permanent location and team will be.
