We are looking for a Care Coordinator RN to join our team in the Welcome Center. Here they will identify and provide case management to international and out-of-area patients. Through relationship development, the care coordinator is responsible for establishing a family centered plan of care aimed at achieving the highest level of wellness. The care coordinator acts as a resource for the family, healthcare providers and community to facilitate collaboration, assist with self-management, manage transitions and mobilize necessary resources. Ongoing monitoring and follow-up is provided through regular communication with patient and family through attendance at scheduled appointments, participation in community meetings, home visiting and/or frequent telephone contact.
