If you are an organized, motivated RN with excellent communication skills and a passion for helping others, we’re saving a case management position for you! The purpose of the Case Manager job is to support the physician, primary medical homes, and interdisciplinary teams in facilitating patient care to improve the quality of clinical outcomes and patient satisfaction. You’ll also be managing the cost of care, and providing timely and accurate information to payors. The role integrates and coordinates functions like resource utilization management, care facilitation and discharge planning. You’ll also help drive change by identifying areas where performance improvement is needed (e.g., day to day workflow, education, process improvements, patient satisfaction). As a case manager, you’re responsible for coordinating a wide range of self management support and for providing information to update and maintain relevant disease registry activity. You’ll be accountable for a designated patient caseload and will need to plan effectively in order to meet patient needs across the continuum, provide family support, manage the length of stay, and promote efficient utilization of resources.
